Aarris Healthcare is seeking compassionate and experienced Per Diem Medical Social Workers (MSWs) to provide home health services throughout the greater Sacramento area including Sacramento, Elk Grove, Roseville, Rocklin, Folsom, Citrus Heights, Carmichael, Rancho Cordova, Davis, Woodland, and surrounding communities.

This position is ideal for social workers who are passionate about helping patients and families navigate healthcare challenges while supporting emotional well-being, community resource coordination, and safe care planning in the home setting.

Flexible per diem schedules available.

Responsibilities

• Conduct psychosocial assessments for home health patients
• Evaluate emotional, social, financial, and environmental needs
• Assist patients and families with coping strategies and adjustment to illness
• Coordinate community resources and supportive services
• Provide short-term counseling and emotional support
• Assist with long-term planning, placement options, and advance care planning
• Collaborate with nurses, therapists, physicians, and interdisciplinary teams
• Educate patients and caregivers regarding available resources and benefits
• Maintain timely and accurate EMR documentation
• Ensure compliance with Medicare and home health regulations

Qualifications

• Master’s Degree in Social Work (MSW) from an accredited program
• Current California licensure/certification if applicable
• Minimum 1 year of healthcare or home health experience preferred
• Home Health experience strongly preferred
• Knowledge of Medicare home health regulations preferred
• Excellent communication and interpersonal skills
• Ability to work independently in the field
• Reliable transportation and valid California driver’s license
