Responsibilities
- Provide direct nursing care to patients requiring ER-level assessment and acute symptom management within a home-based hospice setting, including initial assessments, triage, and ongoing monitoring.
- Administer medications and IV therapies per physician orders; manage infusion devices as applicable; document administration in the designated system.
- Document patient care in Cerner and ensure medication dispensing is performed via Omnicell; maintain accurate MARs and progress notes.
- Collaborate with the care team, including on-call intensivists and hospitalists as appropriate; communicate patient status and care plans to patients, families, and caregivers.
- Ensure patient safety and comfort with pain and symptom management, education on care plans, safety at home, and discharge planning as appropriate.
- Maintain compliance with infection control, privacy regulations, and organizational policies while delivering care in a travel assignment.

Requirements
- Active registered nurse license in California (in good standing)
- Experience with Cerner documentation system and Omnicell medication system is preferred
- Ability to work a 3x12-hour night shift on a travel assignment
- Demonstrated ability to work in a home healthcare - hospice setting and adapt to variable environments
