Home Health RN Job Details

• Employment Type: Travel Contract
• Location: Eureka, California 95501
• Healthcare Setting: Home Health
• Shift: Days, Monday – Friday, 8:00 AM – 5:00 PM
• Hours per Week: 40
• Assignment Duration: 13 weeks (Start: 09/29/2025 – End: 12/28/2025)
• Estimated Weekly Pay: $2320 - $2380 per week
• Patient Ratio: 1:1 (up to 6 patients per day; 30 productivity points standard)
• Travel Radius: Occasional maximum radius of 50+ miles; patient visits are grouped to minimize drive time

Registered Nurse Qualifications

• Active California Registered Nurse (RN) license
• BLS certification (American Heart Association preferred)
• Minimum 1 year of recent skilled Home Health RN experience (within the last 3 years)
• Proficiency with OASIS documentation for start of care
• Reliable transportation required for travel nursing assignments

Home Health RN Responsibilities

• Deliver skilled nursing care to geriatric patients in their homes
• Conduct comprehensive patient assessments, develop individualized care plans, and document using OASIS
• Collaborate with interdisciplinary healthcare team members to coordinate patient care
• Educate patients and families on disease management, medication administration, and home care procedures
• Travel between patient homes within the assigned radius, providing timely and compassionate clinical support
