Position Summary
The Community Health Worker (CHW) serves as a trusted liaison between KCC's program participants and health and social service systems in San Francisco. Working closely with individuals and families who face barriers to care — including housing instability, language access challenges, chronic illness, and economic hardship — the CHW provides navigation, education, advocacy, and direct support to improve health outcomes and quality of life.
This is a community-facing, relationship-centered role that requires a combination of lived experience, cultural humility, and practical knowledge of local resources. The ideal candidate is passionate about health equity and brings compassion, flexibility, and resilience to every client interaction.
Key Responsibilities
Client Outreach & Engagement
- Conduct community outreach to identify and enroll eligible individuals and families in KCC programs
- Build trusting, long-term relationships with program participants in their homes, clinics, community centers, and other community settings
- Conduct health needs assessments and develop individualized care plans in collaboration with clients and care teams
Health Education & Navigation
- Provide culturally and linguistically appropriate health education on topics including chronic disease management, preventive care, maternal health, mental health, and substance use
- Assist clients in scheduling and attending medical appointments, including primary care, behavioral health, and specialty services
- Help clients understand their diagnoses, treatment plans, medications, and self-management strategies
- Navigate clients through complex health and social service systems, including Medi-Cal, Covered California, SF Health Network, and other benefit programs
Social Needs & Resource Connection
- Screen for social determinants of health (SDOH) including food insecurity, housing instability, transportation barriers, and domestic violence
- Connect clients to appropriate community resources such as food pantries, housing services, legal aid, utility assistance, and childcare
- Coordinate with social workers, care managers, and community partners to ensure continuity of care
Advocacy & Systems Navigation
- Advocate for clients' rights and needs within healthcare and social service settings
- Accompany clients to appointments and assist with communication between clients and providers when needed
- Support clients in completing applications for benefits, insurance enrollment, and other entitlement programs
Documentation & Reporting
- Maintain accurate and timely case notes, client records, and activity logs in KCC's electronic health record (EHR) and case management systems
- Collect and report program data including client demographics, services delivered, and outcomes
- Participate in team meetings, case conferences, and quality improvement activities
- Complete required documentation to support grant compliance and program evaluation
Minimum Qualifications
- High school diploma or GED required; some college coursework in health, social work, or a related field preferred
- Minimum 1–2 years of experience as a Community Health Worker, patient navigator, health educator, peer advocate, or in a related community-based role
- Strong understanding of San Francisco's health and social service landscape, including community resources, public insurance programs, and local safety-net systems
- Demonstrated ability to work effectively with diverse, low-income, and medically underserved populations
- Proficiency with basic computer applications (email, data entry, Microsoft Office or Google Workspace)
- Willingness to travel throughout San Francisco for community outreach and client visits
Preferred Qualifications
- Completion of a formal CHW certification program (e.g., City College of San Francisco CHW Certificate, CPEHN-aligned training, or equivalent)
- Bilingual or multilingual proficiency strongly preferred; Spanish, Cantonese, Mandarin, Filipino/Tagalog, Vietnamese, or other languages spoken by SF communities highly valued
- Lived experience with the communities served by KCC, including personal experience navigating healthcare, housing, or public benefit systems
- Familiarity with trauma-informed care, motivational interviewing, or harm reduction principles
- Experience using electronic health records (EHR) or case management software (e.g., eClinicalWorks, Salesforce, Apricot, or similar)
- Knowledge of chronic disease management, mental health first aid, or health literacy best practices
Compensation & Benefits
KCC is committed to supporting the wellbeing of our team members. We offer a competitive compensation and benefits package:
- Hourly pay rate of $25.00 – $30.00, commensurate with experience (DOE)

Work Environment & Physical Requirements
This position is primarily field-based and community-facing. The CHW will work in a variety of settings including community health centers, clients' homes, shelters, schools, and public spaces. The role requires the ability to:
- Walk, stand, and travel throughout San Francisco neighborhoods on a regular basis
- Lift and carry materials (e.g., health education supplies, food items) up to 25 lbs occasionally
- Work a flexible schedule, including occasional evenings or weekends, as required by program needs
- Maintain confidentiality and professional boundaries in all client interactions
