Physical Therapist - Travel Contract

Minimum Requirements

• Doctor of Physical Therapy (DPT) degree required for licenses obtained on or after January 2016; foreign equivalency accepted where applicable.
• Active California Physical Therapy license required to start the assignment.
• Minimum one year of home health physical therapy experience preferred; candidates without home health experience may be considered and trained.
• Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS or equivalent) certification from an approved vendor required.
• Valid driver’s license and must be at least 21 years of age required. Candidates must be able to travel within service area including Butte, Yuba and Sutter Counties; primary need currently in Butte County.

• Facility Type: Health System.
• Position: Therapy - Physical Therapist, Home Health.
• Shift Name: Days.
• Shift Pattern: Monday - Friday, 5x8 schedule.
• Weekly Hours: 40 hours.
• Service Area: Staff will travel to patient homes across Butte, Yuba and Sutter Counties; mileage reimbursement provided separately.
• Assignment Duration: 13 weeks.
