Physical Therapist Job Details

• Location: Irvine, CA 92612 (Orange County, Southern California)
• Setting: Hospital Allied Unit & Home Health
• Employment Type: Travel Physical Therapist
• Assignment Duration: 13 weeks (Start: 09/29/2025 – End: 12/28/2025)
• Schedule: Monday to Friday, 8:00 AM – 5:00 PM (8-hour shifts)
• Hours per Week: 40
• Caseload: Geriatric patients, up to 6 per day
• Productivity Standard: 30 productivity points
• Travel Radius: Up to 50+ miles (clients grouped to minimize drive time)

Physical Therapist Qualifications

• Active California Physical Therapist (PT) license
• At least 1 year of recent skilled home health experience (within the last 3 years)
• OASIS documentation experience required
• BLS certification (American Heart Association)
• Reliable transportation for travel assignments

Physical Therapist Responsibilities

• Deliver high-quality physical therapy services to women's health, vestibular, pelvic rehab, and geriatric patients in hospital and home health settings
• Conduct comprehensive patient assessments and develop individualized care plans
• Implement therapeutic interventions and monitor patient progress
• Complete OASIS documentation for start of care and ongoing visits
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
• Travel to patient homes within the assigned radius, ensuring timely and efficient care
• Maintain productivity standards and accurate clinical records
