The Case Manager (CM) plays a vital role in ensuring exceptional care and operational excellence across up to three residential facilities, supporting a total of 18 clients. Under the direction of the Qualified Intellectual Disabilities Professional (QIDP) or Administrator, the CM oversees all aspects of client support, staff supervision, training, compliance, and financial management. This leadership position ensures that every individual served receives compassionate, person-centered care aligned with Home of Guiding Hands’ mission and values.

Primary Responsibilities

  • Coordinate and monitor comprehensive services for individuals in a home care setting to ensure health, safety, and quality of life.
  • Participate in Interdisciplinary Team (ID Team) meetings to develop, review, and update Individual Service Plans (ISPs) in alignment with client goals and assessed needs.
  • Coordinate client medical care, including appointments, medications, dietary needs, and follow-up care.
  • Maintain accurate, timely documentation and data collection to support service delivery, outcomes tracking, and regulatory compliance.
  • Report and document incidents, injuries, illnesses, and medication changes in accordance with agency policy and state regulations.
  • Supervise and support House Managers (HMs) and Direct Support Professionals (DSPs) to ensure consistent, person-centered care.
  • Collaborate with the QIDP to prepare, deliver, and follow up on Disciplinary Action Reports (DARs) as needed.
  • Coordinate staffing schedules and coverage to ensure adequate support for client needs.
  • Conduct routine health, safety, and compliance audits to ensure adherence to agency standards, labor laws, and state licensing requirements.
  • Support licensing visits, audits, and corrective action plans in collaboration with leadership and the QIDP.
  • Oversee assigned client funds and house budgets to ensure accountability and compliance with financial policies.
  • Review payroll and scheduling documentation to verify accuracy and alignment with staffing plans.
  • Provide intermittent hands-on client support as needed to maintain continuity of care.

Qualifications

  • Must be 18 years of age or older
  • Must have a valid California Driver’s License, safe driving record, and have at least 2 years of licensed driving experience.
  • Must have a High School Diploma or GED; post-secondary coursework in Human Services or related field preferred.
  • Must have at least 2 years of supervisory or leadership experience, demonstrating the ability to guide and motivate staff effectively.
  • Must have the ability to read, write, and communicate clearly in English, both verbally and in writing.
  • Must be CPR and First Aid certified
  • Must be Pro-Act (Professional Assault Crisis Training) certified
  • Must be medication administration certified
  • Must pass a pre-employment physical examination and negative Tuberculosis (TB) screening, with annual updates as required.

Physical Requirements/Work Environment

This job operates in a residential home environment. The employee is frequently required to stand, walk, and sit. The employee may frequently move to interact with fellow employees and/or clients. Additionally, the individual may be required to lift objects weighing up to 50 pounds. Specific abilities required by this job include utilizing wheelchairs, gait belts, Hoyer lifts, and using the buddy system to lift over 50 pounds.
