The Lincoln Families’ Project Permanence program provides supports for youth to stay in their community with their families and prevent out of home placement. Our staff deliver intensive culturally responsive, trauma-informed home-based supports through four phases of service:
- Family Engagement: Identifying the family’s strengths and cultural experiences to determine together specific community support needs;
- Goal Setting: Working together to develop and implement an individualized plan of care designed to meet the goals set by the youth and family;
- Plan Implementation: Supporting the family in driving their plan and effectively meeting their goals;
- Transition: Celebrating the family’s success and strengthening links to identified community supports for sustained stability and permanence.
Youth who have been in foster care, the juvenile justice system – or both – typically face great challenges as they transition back to their families. Many have experienced trauma and have been completely removed from family and their community, which has a significant impact on their ability to trust and have healthy relationships. Repairing and building relationships within the family and community is critical to well-being and a successful transition to a stable and permanent family environment.
POSITION SUMMARY: The Project Permanence Clinical team partners with Youth and Families to implement the High Fidelity Wrap Around Evidence-Based Model by providing direct clinical services, aftercare, and coordinated care in community based settings. Reporting to the Clinical Manager or Clinical Supervisor, the clinician is responsible for conducting the intake process, providing ongoing assessments, offering one-on-one support, and ensuring the Project Permanence team has a comprehensive understanding of participants' mental health needs.
ESSENTIAL FUNCTIONS:
- Facilitates the intake process.
- Obtain and complete necessary clinical records/documents within assigned time frames.
- Provide individual therapy, family therapy, group therapy.
- Assess psychotherapeutic and case management needs of youth and families and collaboratively develop a treatment plan.
- Ensure culturally sensitive and relevant quality of care.
- Develop and monitor working goals and objectives with measurable outcomes.
- Complete outcome measurements within specified time lines.
- Provide crisis intervention to children and families in collaboration with the treatment team. This includes participating in the crisis phone rotation with other clinicians.
- Develop agreed upon discharge plan and assess continuing care needs in collaboration with the Clinical Manager, participants, and treatment team.
- Participates in initial, ongoing, and final case conferences to discuss progress and coordinate care with the Project Permanence team.
- Establish, provide, and facilitate coordinated care with various service providers, on behalf of and based on youth/family specific needs.
- Collaborate with other involved agencies.
- Meet agency expectations regarding levels of direct service.
- Meet required deadlines for documentation.
- Participate in treatment reviews, IEPs and other case conferences as needed.
- Participate in weekly supervision, individual and group, as assigned.
- Requires driving to conduct meetings at homes, schools or other sites and may include transporting youth or families. (Clinician must have a valid CA driver's license, personal automobile insurance and satisfactory driving record.)
POSITION REQUIREMENTS: A positive, service-oriented team player with adept skills in providing outreach, case management, and individual/family support who thrives in a fast-paced environment and is organized to handle often competing priorities. Has a demonstrated awareness of, sensitivity to and competence in dealing with cultural and socioeconomic diversity of client population; able to provide strengths-based, trauma-focused care and dedication to practicing cultural humility in all relationships. Must have strong organizational and time management skills with the ability to work a flexible schedule, including evenings when necessary. Excellent interpersonal and communication skills (verbal and written) with proven ability for complex problem-solving, teaming, planning, and conflict resolution. Must be accurate, detail-oriented and able to maintain a high level of confidentiality. Ability to hold clinical needs of the Project Permanence team. Must have ability to work independently and collaboratively with members of inter-disciplinary teams, seek out internal consultative resources as appropriate, and exhibit autonomous decision-making. EXPERIENCE AND EDUCATION:
- M.S.W. or M.A./M.S. in relevant field, from an accredited graduate school program
- Current BBS registration as associate or BBS licensure
- A minimum of one years’ post-graduation experience in providing direct clinical services to youth/families with a range of mental health needs
- Experience working in an WRAP Around/Team based model is preferred
- Experience with Medi-Cal documentation requirements desirable
- Must meet eligibility requirements to bill for Medi-Cal services
- Bilingual (English/Spanish) language skills highly encouraged
- Basic-to-intermediate computer proficiency
