Job Summary:
Physical Therapist Outpatient (PT OP) position located in Ukiah, California. This role involves developing and implementing treatment programs to improve patients' physical, social, and mental functions within an outpatient setting. The therapist will serve as a co-leader in an interdisciplinary team to provide comprehensive care.

Job Details:

• Schedule: Day shift, 8 hours per day, minimum of 40 hours per week.
• Required Qualifications: Doctor of Physical Therapy (DPT) degree; licensure in the state of practice; CPR or Basic Life Support (BLS or HS-BLS or RQIBLS) certification.
• Preferred Qualifications: Pelvic floor therapy experience, PRPC or WCS certification or working towards certification, 3 to 5 years of experience.
• Additional Information: Responsible for completing evaluations, establishing individualized treatment plans, providing therapy interventions, maintaining comprehensive clinical documentation, supervising assigned assistants, and performing other related duties. The position begins on 07/20/2026 and is located at 275 Hospital Drive, Ukiah, California.
