Experienced LVN Needed for PM Coverage

We are seeking an experienced Licensed Vocational Nurse (LVN) to provide evening and night shift coverage in a home health care and private duty setting. This role requires effective patient communication and offers flexible scheduling for those PM shifts.
Urgent needs include:
- FT availability (Mon-Fri) 3p-11p and 10/11p - 6/7a
- PT availability (weekends)
- PT availability (1-3 days/week)
Pay Range: $28 - $32

Responsibilities
- Communicate effectively with patients to assess and address their health needs
- Provide skilled nursing care in home health and private duty environments
- Coordinate with healthcare team members to ensure continuity of care
- Document patient information accurately and timely
- Adhere to established healthcare protocols and safety standards
Required Qualifications
- Minimum of 1 year experience as a licensed vocational nurse
- Valid LVN license
Preferred Qualifications
- Experience in patient care
- Strong communication skills
