Job Title: Case Manager Profession: Registered Nurse Specialty: Case Manager Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours Experience: Minimum of 2 years of experience required License: Valid nursing license required Certifications: BLS Must-Have: - Previous charge experience preferred - Experience with charting systems - Experience in acute hospital settings - Experience with various patient demographics, including adults and geriatrics - Ability to float between units as needed - Must have current work history within the last 2 years Description: The Case Manager will collaborate with healthcare teams to ensure effective patient care coordination. Experience in case management is necessary, along with familiarity with CMS guidelines and benefits eligibility. The candidate will participate in the development of treatment plans and discharge planning. Ability to arrange durable medical equipment and transportation services is required. Competence in managing disease management and insurance company processes is expected. The position involves managing patient care across various settings and potentially floating between facilities as necessary. Knowledge of coding and billing, including ICD 10 and CPT, is preferred. The candidate must ensure compliance with HIPAA regulations. This role may require working weekends and holidays. References are required covering experience within the last three years.
