Position Summary
A Direct Support Coordinator works as scheduled by the Regional Manager and/or his/her designee. His/her schedule may be either part or full time, depending on the needs of the clients and Adjoin. Overtime may be required as dictated by operational needs; however, no holiday or overtime may be worked without the prior approval of the Direct Support Coordinator's supervisor.

Successful completion of pre-employment drug screen and background check. Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test. Acceptable DMV record. Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.

Facilitate the client’s inclusion in positive community experiences. Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel.

Advance client’s decision-making, productivity and participation in a range of daily activities. Perform all other job duties as assigned.

Assist
Promote independence in daily activities yet be mindful of client’s intellectual and physical disabilities and assist with meals, cooking, household maintenance, grooming and dressing, as assigned. Assist with training employees to perform their direct support duties in accordance with the policies and procedures of Adjoin.

Maintain current knowledge of client’s needs. Maintain positive communications and relationships with his/her supervisor, clients, landlords, employers, involved members of families, Regional Center, D.O.R. and other individuals in the community.
