Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, aligned with department objectives
- Assess patients, develop and execute individualized treatment plans, and monitor progress toward established goals
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating with other professionals to optimize outcomes
- Document patient assessments, treatments, and outcomes in accordance with applicable guidelines and regulations
- Educate patients and families on therapy plans, home exercises, and safety practices to support continued progress
- Ensure adherence to safety, quality, and regulatory standards throughout the treatment process

Requirements
- Active Physical Therapy license in California or eligibility to practice in the state
- Ability to work in a home-based care setting and collaborate with an interdisciplinary team
- Strong communication and documentation skills
- Demonstrated ability to contribute to patient-centered care plans and group treatment activities
- Willingness to work a day shift schedule and fulfill a 40-hour workweek as assigned
