Responsibilities
- Provide occupational therapy evaluation and treatment planning for patients with SNF needs, aiming to improve activities of daily living, mobility and functional independence.
- Implement evidence-based OT interventions, adapt plans to patient progress, and document outcomes to inform discharge planning.
- Collaborate with the interdisciplinary team (nursing, therapy, social work) to coordinate care and transitions across inpatient and outpatient services.
- Maintain patient documentation in PowerChart and update any required logs; participate in daily huddles to coordinate outpatient clinic coverage (2–4 hours) as needed.
- Monitor safety, assist with ADL retraining, cognitive activities, and upper-limb function as part of comprehensive rehabilitation.
- Scheduling is dynamic based on patient load; weekend coverage for OT is determined by daily needs and is not guaranteed.

Requirements
- Ability to work in a dynamic health care setting with a focus on SNF rehabilitation.
- Demonstrated capacity to collaborate with multidisciplinary teams and adjust interventions based on patient progress.
- Proficiency in documentation and charting, including use of an electronic health record and maintaining a manual log as required.
- Scheduling is typically 8:30 a.m. start with a daily 30-minute lunch, with variations based on patient needs; 5x8 workweek.
