Job Title:

Occupational Therapist (OT) & Physical Therapist (PT) — PRN / 1099 Contractor or Part‐Time W2

Setting:

Rehab Facility / Adult Post‐Acute Care

Schedule:

Flexible PRN; weekday, weekend, half‐day, and full‐day options available

Work Model:

On‐site patient care with remote completion of eligible documentation duties as permitted by DOR.

Compensation:

$72/Hr depending on experience and qualifications

Duties

Occupational Therapist (OT) Duties

• Evaluate and treat adult patients with ADL deficits, cognitive/perceptual needs, upper‐extremity limitations, and functional impairments

• Develop and implement individualized OT treatment plans

• Provide interventions including ADL retraining, adaptive equipment training, UE strengthening, and safety education

• Document evaluations, treatment sessions, progress notes, and discharge summaries

• Collaborate with rehab, nursing, and interdisciplinary staff

• Educate patients and caregivers on home programs and functional carryover strategies

• Maintain timely, accurate, and compliant documentation

Physical Therapist (PT) Duties

• Evaluate and treat adult patients with mobility, gait, balance, strength, and functional movement impairments

• Develop and implement individualized PT treatment plans

• Provide interventions including gait training, therapeutic exercise, neuromuscular re‐education, and transfer/balance training

• Fit and train patients on assistive devices (walkers, canes, wheelchairs)

• Document evaluations, treatment sessions, progress notes, and discharge summaries

• Collaborate with rehab, nursing, and interdisciplinary staff

• Educate patients and caregivers on mobility strategies and fall‐prevention programs

• Maintain timely, accurate, and compliant documentation

• Active California OT license

• Adult rehab / SNF / post‐acute experience preferred

• Strong clinical documentation skills

• Ability to work independently and maintain professional, compliant standards

• Adult rehab / SNF / post‐acute experience preferred

• Strong clinical documentation and mobility‐focused assessment skills
