INTAKE SERVICE SPECIALIST**

FAMILY SOURCE CENTER**

SUMMARY OF DUTIES**:

Performs intake and eligibility tasks in enrolling clients into the Family Source Center program. Provides crisis intervention and supportive services to clients in need of emergency services. Create intensive services profiles and conduct appropriate case documentation, soap notes, and follow-up with clients. Performs regular upkeep of files and inputs client and program data into database on a regular basis. Assist clients in finding resources and other services

EXAMPLES OF DUTIES AND RESPONSIBILITIES: **

Processing Intake and Eligibility form for clients.

Inputs data on the City's database program.

Create Crisis Intervention pre-assessment profile

Maintains all forms and documents necessary to enroll new clients.

Provide Crisis Intervention services to emergency need clients

Performs other duties as assigned by supervisor

Communicate with DCFS Social Workers, LA County Probation Officers, City Prosecutor's office and LAUSD School Police on behalf of clients.

Perform DCFS drug Testing on mandated clients

Work Schedule Monday - Friday 8:30am to 5:30pm

QUALIFICATIONS AND EXPERIENCE:**

Minimum two years of the following experience: crisis intervention, customer service, program and file notes, heavy phones, rapid working environment, fast-paced office setting, low-income populations, and file management.

Must possess a friendly attitude and be dependable; must be able to function in a busy office setting. Computer literate, knowledge of Word and Excel software.

EDUCATION:**

High School Degree and ability to write and perform clerical functions. Evidence of prior experience working with low-income populations and customer service desirable.
