• You will coordinate the care and service of the patient population from admission through discharge.
• You will assess and Plan, implement, monitor, and evaluate options and services to meet an individual's health needs to facilitate a safe and timely discharge to the next appropriate level of care.

Qualifications Qualifications: Education:

• BSN required

License/Certification:

• Current, unrestricted California RN license
• American Heart Association BLS
• Case Management certification preferred or willing to obtain within a year from start date

Experience:

• A minimum of 3 years Case of Management experience preferred
• At least 2 In acute care nursing - Medical Surgical/Tele/ICU/LTACH or 1 year inacute care nursing (Medical Surgical/Tele/ICU) with 2 or more year of care management experience (UM or DC Planning).
• Knowledge of medical literature, research methodology, health care delivery systems and financial reimbursement required
