Were seeking a Board-Certified or Board-Eligible Physician in Internal Medicine Family Medicine or Geriatrics to deliver comprehensive primary care to aging adults across home-based and community settings in San Diego CA .

What Youll Do

• Serve as the primary care provider for assigned patients managing preventive acute and chronic health needs.

• Perform comprehensive health assessments order and interpret diagnostics and guide personalized care plans.

• Collaborate with an interdisciplinary team of Nurse Practitioners Physician Assistants and Wellness Coordinators .

• Lead population health initiatives and quality improvement programs that enhance patient outcomes.

• Provide mentorship to mid-level providers and clinical support staff.

• Participate in regular provider meetings and professional development to advance clinical excellence.

Who You Are

• MD or DO with an active California medical license .

• Board Certified in Family Medicine Internal Medicine or Geriatrics.

• Experienced in senior or geriatric care (preferred).

• Skilled in EMR documentation and team-based collaboration.

• Compassionate communicative and driven by patient-centered outcomes.
