Mission Pediatrics - MA/Referral Coordinator. Salary: $23-$27 per hour | $1,000 signing Bonus. Location:   ~6950 Brockton Ave., Ste. 4, Riverside, CA 92506. Looking for a vibrant MA or Referral Coordinator with Pediatric experience that can help coordinate care and bring extensive support to our patients. JOB DESCRIPTION: A Care Coordinator within the Enhanced Care Management (ECM) Program at Mission Pediatrics plays a crucial role in providing high-quality care management to improve the overall health and well-being of pediatric members and their families. Operating under the general supervision of the Enhanced Care Management Department Leadership, you will collaborate within the ECM team, supporting and coordinating the healthcare of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other specialists. TASKS: Engage patients, inform them of their needs, and empower them and their families to achieve understanding of their chronic disease management process. Screen patients for medication adherence, social determinants of health, and physical needs. Facilitate patient engagement and follow-up care, including assistance in accessing specialists, ancillary services and/or transportation as needed.  Assist the patient with referral inquiries. Provide patient education about chronic disease management, as well as available financial, social, and community resources. Monitor and take responsibility for the assigned patient population. Ensure preventative measures are performed within the published periodicity timelines. Systematically track treatment response and monitor patients (by telephone or electronically) for changes in medication adherence, access to care and care plan. Provide resources when appropriate or requested. Collaborate with providers to assist in initiating an individual care plan with goals for patient. Track and record patient follow up and clinical outcomes.  Document telephone encounters in the care plan and use the system to identify and re-engage patients.  Enter and document patient related data within the EMR system. Identify and report to manager opportunities to improve efficiencies in tracking and management of these patients.  Performs other duties as assigned.
