Responsibilities
- Perform initial evaluations and ongoing reassessments for outpatient physical therapy patients.
- Develop, implement, and adjust individualized plans of care focused on functional goals and musculoskeletal rehabilitation.
- Deliver evidence-based interventions, including therapeutic exercise, gait training, manual therapy, and modality application as appropriate.
- Monitor patient progress, document in the electronic health record, and communicate findings with the supervising clinician and care team.
- Educate patients and families on home exercise programs, activity modification, and safety to support discharge planning.
- Maintain patient safety, professional standards, infection control, and compliance with privacy and regulatory requirements.

Requirements
- Active Physical Therapy license in California.
- CPR or Basic Life Support (BLS) certification from an AH-approved vendor.
- Ability to work in an outpatient physical therapy setting within a hospital environment.
- Travel assignment eligibility and ability to meet scheduling requirements for a 13-week assignment starting 07/06/2026.
