In this dual role, you will serve as a key clinical leader for the hospice program, providing oversight of clinical quality, hospice eligibility determinations, symptom management, care planning, and regulatory compliance while also delivering direct patient care and consultation. You will collaborate closely with an interdisciplinary team of nurses, social workers, chaplains, and other healthcare professionals to develop individualized plans of care that support patients and families throughout their hospice journey. The Hospice Medical Director / Team Physician also serves as a mentor and resource to physicians and clinical staff and fosters strong relationships with attending physicians, hospitals, long-term care facilities, and community partners. This position combines the opportunity to make a meaningful impact at the bedside with the ability to shape clinical excellence and advance hospice care across the organization. The ideal candidate is a collaborative, compassionate physician with expertise in hospice and palliative medicine, a commitment to interdisciplinary teamwork, and a passion for improving the end-of-life experience for patients and families.
