Mobile Care Specialist 1

Compensation: $21-25/Hr.
Position Summary:
The Mobile Care Specialist I (MCS1) will be directly extending at-home patient care, under direction of the remote physicians within the Falck Physicians Group’s Mobile Care Program (Falck Mobile Care).
Falck Mobile Care offers same day urgent care and health care services directly to patients’ homes throughout Los Angeles County and Orange County. As a MCS1, your role will be to deliver high quality patient care in non-clinical environments, take vital signs and collect intake information, assist with point of care testing and therapeutics, manage remote patient/physician contact via telehealth equipment, answer questions regarding mobile care services, complete in-home safety and care assessments, gather relevant patient information, and facilitate communication between other members of the Mobile Care team. The MCS1 will also be responsible for organization and administration of provider instructions, EHR updates, member demographics, and patient follow-up.
Essential Functions:
Mobile Care Specialist Tasks Include:
- Drive the Mobile Care company vehicle to and from patient’s desired visit locations
- Verify patient identity, insurance coverage, and other relevant medical information preceding care
- Maintain clear lines of communication with remote team, such as Care Coordinators and Providers
- Explain purpose of all assessment and treatment procedures to patients and caregivers for their own comfort and understanding
- Use telehealth equipment to record and transmit vital signs and relevant images or recordings
- Ensures accurate notes of all consultations and treatments are recorded in the patients’ EHR
- Manage IT workflow and connectivity before starting virtual face-to-face visit with remote Provider
- Under direction of remote Provider, collect lab specimens and perform point of care testing
- Oversee patient and caregiver education regarding prescribed treatment, medication pick-up, test result timelines, and any care escalations
- Administer medication, change wound dressings, and care for other treatment options
- Activate emergency response systems during medical emergencies or other crisis
Essential Responsibilities
- Upholds Falck Physicians Group Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws
- Addresses and escalates patient requests and concerns regarding care
- Assists in equipment management and reports any problems regarding stocking, vehicle maintenance, and IT in an effective time frame
- Manage onsite authorization processes for treatment consent, information sharing, and insurance verification
- Ensures seamless transition of information between patient and remote care team members
- Provides direct oversight of patient education, preparation for home visits, plan of care initiation, and coordination of care with remote/deployed care team members
- Be able to access medical records and respond to insurance inquiries on past encounters
- Respond accurately to patient and caregiver questions regarding pharmacy, lab results, and follow up calls
- Works collaboratively with members and staff across all service lines
- Supports and demonstrates Falck Physicians Group’s customer service philosophy and manages patient interactions and internal communication in a professional manner
- Identifies and/or proposes ways to improve customer service
- Demonstrates an awareness and sensitivity to patient/family rights
- Handles inquiries and complaints pursuant to procedure
- Complies with departmental standards, policies and procedures regarding training, injury prevention, management of workload, and safety/emergency situations
- Performs other duties as required

Qualifications:
- Completion of EMT / Medical Assistant Training Program
- CPR and BLS Certification
Experience:
- California Medical Assistant / EMT
- High School Diploma / GED
- Current state driver’s license
- Satisfactory driving record in accordance with the Falck USA Driving Policy
- Customer service and healthcare related experience
- Minimum of two years of direct painter care
- Preference for blood withdraw training
- Preference for additional experience with healthcare administration or 911 dispatch
- Preference for Spanish language proficiency

Work Schedule:
- Evenings, could include flexing to Day and Night Shifts
- Monday to Friday (could include Weekends and Holidays)
- Working Hours: 2 pm to 10:30 pm
