Physical Therapist - Travel Contract

Minimum Requirements
- Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active California Physical Therapist license required prior to the start of the assignment.
- BLS certification from AHA or ARC may be required.
- No new graduates. Previous outpatient physical therapy experience is required.
- Experience with functional treatment programs, patient evaluation, goal writing, and documentation in accordance with clinic and payer guidelines.
- Ability to complete timely and accurate medical record documentation, including letters of medical necessity and charge entry.
- Demonstrated competency in safe patient handling and materials handling procedures.
- Willingness to participate in student supervision and professional development activities.
- Adherence to state practice act, professional codes of conduct, and maintenance of required continuing education.

- Facility Type: Outpatient.
- Shift Schedule: 5x8.
- Assignment Duration: 13 Week.
- Full-time, 40 hours per week.
- Start: ASAP.

Provide evaluations and individualized treatment plans to progress patients toward discharge goals. Maintain clinical documentation and comply with physician orders and insurance authorizations.
