Home Health Physical Therapist (PT) - Per Visit Pay Up to $155/visits

- Valid California Physical Therapist license
- CPR or BLS certification
- Driver's license and auto insurance
- Physical + TB test (within the last year)
- Professional liability and business license (if applicable)
