Responsibilities
- Deliver direct nursing care to assigned patients in a home hospice setting using established standards and the nursing process.
- Assess patient data, develop and implement a care plan, monitor progress, and adjust interventions as needed.
- Collaborate with the interdisciplinary team to coordinate care, support safe transitions, and contribute to discharge planning as appropriate.
- Educate patients and families on disease processes, symptom management, medications, and end-of-life care planning.
- Supervise and delegate tasks to licensed and unlicensed staff as appropriate, ensuring safe, high-quality care delivery.
- Document care and plan of care in the EPIC EMR; participate in care coordination and ongoing patient education.

Requirements
- Active California RN license
- Minimum 2 years of recent RN experience in a relevant field
- BSN preferred
- Certifications: BLS/CPR and ACLS; HLFS (Hospital Fire and Life Safety) required
- EPIC EMR experience required
- Ability to work in a home healthcare - hospice setting
